Data Protection  


The practice makes extensive use of information technology. Your registration details and some medical history details are held on our computer. Consultations are recorded on the system. It produces all the prescriptions and will help us provide comprehensive health care. If you have any questions, a Doctor or the Patient Services Manager will be pleased to help. We are registered under the Data Protection Act and no one outside the practice has access to data identifiable with you. Anonymised data is made available for research purposes only.  


 Summary Care Records - the last government committed to create a summary care record for every patient and for that data to be loaded onto a national database so that hospitals and GP's could have access to it if a patient presented. It will hold registration data, medication data and major illness/conditions information. Patients are allowed to opt out of this process and can complete a form that is required to be sent back to the practice. Currently the summary care record process is on hold within Ealing as consultations are ongoing between health authority and government. June 2011



Care Data - You may have read the leaflet that should have been delivered to all households in the UK regarding the extraction of data from patients’ records to improve the quality of care and health services for all. If you do not want your information to be shared in this way and would like to ‘opt out’ of the system we can record your dissent by entering the Read code ‘9Nu0 – Dissent from secondary use of GP patient identifiable data’ into your computer record. Please click here to download the form. 


If you wish to view, or want copies, of some or all of your medical records please ask at Reception for a form. We recommend that a healthcare professional be present when you view your records to explain medical terminology and clarify any concerns you may have. There is normally a charge to view your records. There will be a charge if you request photocopies of some of your medical records. Please ask at Reception for more details.
