Registering With Us


Thank you for your interest in registering at Hillview Surgery. If you would like to register at this Surgery you must live in our catchment area. To view a list of roads in our catchment area please click here. You will need to provide proof of this by providing one of the following documents showing your correct name & address. It must be dated within the last three months.

  • A bank or building society statement.
  • A utility bill e.g. gas, electricity or water.
  • A council tax bill.
  • A letter from a Government Dept. e.g. The Inland Revenue.
  • A signed tenancy agreement from the landlord or letting agency.

 We also ask to see photo ID.

  • A passport.
  • An EU ID card.
  • A full driving license card (for UK born residents)
  • A birth certificate for newborns being registered for the first time

If you have registered in the UK before we will need the name & address of your previous GP & your NHS No. This information can be found on your Medical Card if you have one. If not, you will need to contact your previous GP. We cannot process your registration without this information.


If you have lost your Medical Card or have never been registered in the UK please complete this GMS1 form. Please also complete The New Patient Registration Questionnaire  for Adults or The New Patient Questionnaire for 16 years and under. Please return with the completed forms, in person, Monday, Tuesday, Wednesday and Friday between 10.00am - 5.30pm and on Thursday between 10.00am - 12.30pm. (All those over 16 years of age must attend in person, parents/guardians should provide ID for children under 16 years of age.)
If the GMS1 and the Questionnaire are not fully completed your registration application may not be processed that day.
Please allow three working days for your registration to be completed before requesting an appointment.
If you are registering children under 16 years of age we ask that you bring a copy of their Red Book or immunisation history so that their records can be kept up to date. If these are in a foreign language please provide an English translation.


Your named GP will depend on the first letter of your surname:


A-D   =   Dr S Shergill

E-K   =   Dr A Mann

L-Q   =   Dr V Siva

R-Z   =   Dr S Michael


Your named GP will have overall responsibility for the care and support provided by Hillview Surgery. You may still see any other GP of your choice if they are available.f you do not have the necessary documents to register but you need to see a doctor you may be able to have Temporary Registration.  The Receptionist will advise you.
