We would like to take this opportunity to remind any patients who are yet to sign up for online services to do so. You can access online services by registering for the NHS App. Once you are registered you will be able to use the service to:
- Order your repeat prescriptions
- Make a telephone appointment
- Cancel an appointment
Along with ordering repeat prescriptions, and making and cancelling appointments, Airmid allows you to request access to and view your online medical record.
Proxy Access may be requested to another patient’s record but must be supported by signed consent from the patient. Please complete this form Patient Online Proxy Consent Form and hand it in at reception.
Parents/Guardians may request proxy access to their child’s records for children up to the age of 11 years. Patient Online Proxy Consent Form – Under 11 years At 11 years of age proxy access will be withdrawn. At 16 years of age patients should submit their own request.
Online access has been temporarily withdrawn until further notice for patients aged 11 years to 16 years due to the lack of clarity regarding individual access and proxy access for parents/guardians of this age group.
These online services are as safe as online banking as long as you keep your passwords secure, and you can access these services 24 hours a day 7 days a week.
If you already have an account that allows you to book, check and cancel appointments, order repeat prescriptions and look at a summary of your records but you would like increased access to your records, please request this via the Airmid App for iOS or Android, or send an email to email@example.com with the subject “Request for Full Clinical Record Access”, and a member of the team will contact you to arrange this.
It may take up to 30 days before this request is actioned.
